Jackie Griffin, RN, BSN is the Care Coordinator for Tahoe Forest Hospital. She comes to this position after working for the past 9 years on the Medical- Surgical Unit at Tahoe Forest Hospital. Her diverse 20-year nursing experience includes: In- patient Rehabilitation Admissions and Education Specialist, Orthopedic, Telemetry, Neurological and Geriatric Nursing experience as well as Director of Nursing in Long Term Care. She is a graduate of the University of Maryland School of Nursing.

Care Coordinator, Jackie works with people in the community who live with chronic illness. She assists them with navigation of the health care system, improving access to Primary Care Practitioners, Community Resources and Specialists. Care Coordination is a patient- and family-centered, team-based model of care designed to promote self management and improved quality of life. With the help of a multidisciplinary team, Jackie will be facilitating classes in Chronic Disease Self Management and Diabetic Self Management Programs in the community.

Additionally, Jackie is a Mindfulness Based Stress Reduction Program Instructor. She currently teaches this world-recognized and evidenced-based program throughout the Tahoe Area.
