Appointments

Please bring a valid ID and your insurance card to every visit. Because we know your time is valuable, we ask that you extend us the same courtesy and arrive prior to your scheduled appointment time. If another child in your family needs to be seen as well, please call in advance to let us know. Your child will be seen more quickly if you have a scheduled appointment.

We understand you might not be able to keep a scheduled appointment. Please call and cancel to allow the opportunity for another child to be seen. If you are late for your appointment (>15 minutes), we will do our best to accommodate you. However, on certain days it may be necessary to reschedule your appointment.

If you or your child have any of the below signs or symptoms, please try your best to have your child wear a mask. They are located at the entrance of the building. We want to avoid the spread of diseases and many can be spread through air droplets and close contact.

  • Cough
  • Fever with rash

Refilling a prescription’s is at the doctor’s discretion and may require a visit if warranted.

Your child must have a well-child check within the last year in order to have a form completed, UNLESS your child is 11+, every other year is acceptable according to the school district.
