Tahoe Forest Health System offers a convenient and timesaving pre-registration program for pre-scheduled services such as surgery.
After your physician schedules your procedure at Tahoe Forest Hospital or Incline Village Community Hospital, an admitting representative will contact you to confirm your information, as well as verify your insurance information and authorization requirements. A patient chart will then be created, and the admitting process will be completed prior to arrival before your procedure.
When you arrive for your procedure, you may be asked to sign final admission paperwork. Please bring your insurance card and any orders from your physician.
As part of the pre-registration procedure, we will ask that you complete and submit the following information:
- Name, address, and phone numbers
- Date of birth
- Name of employer
- Name of your physician(s)
- Insurance information
To register for services (including Ambulatory/Outpatient Surgery, Diagnostic Services, and Laboratory) at Tahoe Forest Hospital or Incline Village Community Hospital, please enter the Main Entrance and check in with a patient registration representative at the Lobby front desk. They will expedite your registration and help with any questions you might have.
For after-hours, weekends, or in an emergency go directly to the Emergency Department desk and check in with the patient registration representative. Our Emergency Department is staffed 24 hours a day, seven days a week, 365 days a year.
