Overview

Health disparities are largely preventable differences in health outcomes that affect populations facing greater barriers to optimal health, often tied to socioeconomic status. To foster a healthier community, we aim to reduce disparities in health care access, utilization, and in the risks, prevalence, and challenges associated with socioeconomic factors.

Goal

Three metrics for Health Disparities are included in the Community Health Index. The target population for these measures is socioeconomically disadvantaged (SED) community members.

Currently Insured

Goal: Increase the proportion of SED community members who have health insurance

Has a Personal Doctor or Health Care Provider

Goal: Increase the proportion of SED community members who have a primary care provider

Food Insecurity

Goal: Reduce the proportions of SED community members experiencing food insecurity
