Health and Resource Advocates are bilingual healthcare team members who provide advocacy, connections to community resources and interpretation as needed for socially complex patients.  They are able to meet patients in the clinic, office, field or home per patient preference, in addition to the monthly telephone outreaches.  Health and Resource Advocates also provide community outreach for health screening and culturally appropriate education.

Health and Resource Advocates help patients and community members achieve their best health!

  • Provide community outreach and culturally-appropriate education.
  • Lead support groups and workshops.
  • Assist in accessing and navigating medical, social and financial resources.
  • Provide advocacy on behalf of patients and families.
  • Help patients understand and follow their treatment plan.

Who is eligible?

CHA’s work with anyone who has at least two of these needs:

  • Medical needs that require ongoing care
  • Disability
  • Mental health needs
  • Limited health comprehension/health literacy (English and Spanish)
  • Transportation barriers
  • Limited social supports
  • Financial hardships (assistance with food, utilities, medication, health insurance, unemployment, housing, etc.)
