Overview

Chronic diseases are health conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. Chronic disease includes heart disease, cancer, obesity, diabetes and even tooth decay. Risk factors for chronic disease include smoking, poor nutrition, physical inactivity and excessive alcohol use.

Goal

Two chronic conditions are included in the Community Health Index.

Diabetes

Goal: Increase diabetics who have blood sugar in control

High Blood Pressure (Hypertension)

Goal: Increase those with hypertension who have blood pressure in control
