Health Promotores help patients and community members achieve your best health!

Tahoe Forest Health System bilingual Health Promotores are public health workers and trusted community members who support chronic disease management, pregnancy and postpartum care, behavioral and mental health, cancer care, and other high-risk medical needs.

Health Promotores offer the following services:

Provide community outreach and culturally-appropriate education

  • Lead support groups and workshops
  • Assist in accessing medical, social, and financial resources
  • Provide advocacy on behalf of patients and families
  • Help patients understand* and follow their treatment plan

Who is eligible?

Health Promotores work with anyone who has at least two of these needs:  

Medical needs that require ongoing care

  • Disability
  • Mental Health needs
  • Language barriers*
  • Transportation barriers
  • Limited family/friend support
  • Financial hardship (assistance with food, utilities, medication, etc.)
  • Low literacy

*For language assistance (translation) only, patients are linked with the Language Line.
